Submitting forms remotely

There are multiple ways to submit documents to us without having to come to our office. Check your financial aid requirements in the Financial Aid portal in Academica. Many of our forms are electronic and can be submitted to us directly. Documents without social security numbers or other personally identifiable information can also be emailed to us at studentservice@wayne.edu. If your forms are not electronic you can use the link below. Documents submitted via this link will be reviewed and identified before your requirements are updated in Academica.

Withdrawal

• Intent to Attend after Withdrawal form - Complete this form if (1) you are receiving financial aid and (2) you are withdrawing from all current classes, but (3) you are registered in a class/classes that begin later in the semester.

Students with a print disability, a visual limitation or reading difficulty that limits access to traditional print may request assistance from WSU Student Disability Services.

Many forms and charts may be printed. They are in Portable Document Format (PDF) Adobe PDF. Adobe Acrobat Reader software is required to view and print PDF documents. It is available for download, free of charge.
