The Final Assessment is the last phase of the process. Recall that dialogue should have been occurring all along the performance year. At the Mid-Year and whenever there were changes to the original objectives, the documentation should have been recorded in the middle column. There should be no surprises during this Phase.
Keep in mind that the employee needs to update the status of his/her objectives by including:
- Accomplishments
- Barriers
- Rationale for changing any part of the objectives
The employee should also complete other contributions as well.
The employee should feel free to add comments in the Employee's Comments on Part A, Part C of the form. There should be conversations going back and forth between the supervisor and the employee to firm up the status.
The supervisor needs to review the status of the employee's objectives and input own observations and assign a rating for each objectives. The supervisor should plan to use the Supervisor's Comments on Part A and Part C of the form noting the date of comment and then plan to review and comment on Other Contributions on Part A and then a Summary of Performance on Part D. The form should be printed and signed by the supervisor and then reviewed by the second level reviewer and signed.
The Final Assessment should be discussed between the supervisor and employee during the formal Final Assessment Meeting.
After the Final Assessment Meeting, the employee can write in any final comments in Part D of the form and then needs to sign the form to to acknowledge that a discussion has occurred on the Final Assessment.
Copies should be made for all parties and then the original should be sent to the HR/Employment Services Office: 1900 A/AB. Please check with your department to verify the process within your own unit as the Business Manager or Personnel Officer from your unit may want to receive the originals to send all your department's assessments to HR at one time.
