LEADS Form Guide

For Non-Represented Academic Administrators:

Wayne LEADS Form for Performance Year 10/1/2012 - 9/30/2013

For Performance Year 2013 we have updated the form to reflect feedback recieved by users of the process:

  1. Deleted “Phases” and called them “Elements”
  2. Competency Assessment
    • Only takes place at Final Assessment
    • Employee and Supervisor pick the level of skill observed/displayed
    • Added check mark boxes for both Employee and Supervisor to acknowledge discussion between supervisor and employee on all the competencies at the bottom of the page.
  3. Supervisor needs to take level of skill into consideration when assigning final rating although they are not the same words there are the same number of ratings and the top rating for the objectives can be equated with the top level of the skill
  4. No “Mid-year”  -- just “Status Updates”
  5. No Employee Development Plan
  6. No weightings
  7. Final Rating is not Calculated.  Supervisor needs to put in.
  8. Can also be used for probationary/provisional

Once the form is opened...

  1. Save it to your own local drive.
  2. Call it what you want but we recommend using your first name and first initial of your last name with LEADS for easy identification, (example: FirstNameLastInitialLEADS.pdf).
  3. Now instead of using the form from the this website, open your saved form from your computer.
  4. Once the form is open from your computer, you can start entering information into the form. You can save it and reopen later or save it and E-mail as an attachment.
